to Retire

The attached form is a
sample form only and is not intended for
use. The actual 3-part forms which
employees are to use are available at
the Front Reception Desk at the
Administration Building, 701 N. Madison
Street, Stockton, CA 95202. You will
note that this election to retire will
be irrevocable, unless the District
withdraws the SERP Plan, in which case
retirement will be automatically
rescinded.*
*Should the employee wish to continue
with their retirement if the District
withdraws the SERP Plan, they must
contact the Human Resources Department
for instructions as to how to proceed.
Reminder:
The District is
only offering the SERP Plan to eligible
STA, SPPA and USA members. |
